TMNDX forms provide an efficient step ‐by ‐ step intake process for the required individual history and clinical data collection of patients with craniofacial pain, TMD, chronic pain and sleep breathing disorders. The forms were developed out of the need for an intake system to meet the most current literature noting that 90% of those who suffer from craniofacial pain and TMD also have sleep disorders. Although this correlation was identified, most of the available intake forms and systems separated these conditions, focusing on one or the other; Facial pain/TMJ only or Sleep breathing problems only. This left the clinician without a crucial part of the information needed to triage and determine primary diagnosis for treatment direction with long term success TMNDX Software was designed to match the forms with the goal of ensuring consistent and accurate intake of information and data collection for use in the triage process; allowing the dentist to determine and direct treatment to include the appropriate referrals to other healthcare professionals required in an interdisciplinary approach and optimize results for the patients suffering from these conditions.

The software provides a dynamic step‐by‐step format for time efficient intake of information. It provides tracking and storage of the collected data for use in electronic generation of necessary letters, reports and insurance claims to assist the dentist in meeting the medical model for the conditions of craniofacial pain, headache pain, and airway and sleep breathing disorders.
